New Patients

We strive to be respectful of all our patient's time to the fullest extent possible. To that end, taking a bit of time to complete your New Patient Packet prior to your arrival for your consultation will improve our ability to maintain this goal. We appreciate your efforts on this. Within the packet you will find a patient information form, medical release/insurance agreement, financial agreement, and a receipt of privacy policy (the privacy policy is also attached in separate form for your convenience). 

If you are not able to complete the packet at home or prefer to fill it out at the office, please arrive at least 30 minutes prior to your scheduled appointment to allow time for completion of the forms and assistance with any questions you may have. 

Please remember to bring the following items to your appointment:

• Completed new patient packet
• Current drivers license or photo identification
• Insurance card(s)
• Any specialist co-pay and/or co-insurance amounts
• A list of current medications (Name, Dose/Strength, Number of times per day)

There are two options to complete the form pending your preferences (please complete ONLY one): 

1) PDF - Form can be filled out via hand written entirely prior to signing 

2) Coming soon... dotm - Form can be filled in via Microsoft Word prior to signing
