Referrals from a Medical Office
If you are a physician's office and you would like to refer a patient to Coastal Pain & Spine Center, simply download and fax the referral form below. Once received, we will take care of the rest. Our referral coordinators will call and schedule the appointment and we will return fax the date and time of their consultation.
If you have an urgent matter, please feel free to call our office to discuss with either our office manager or one of our physicians. We strive to accommodate a same day or next day appointments for urgent matters. Please also fax the referral form.
We accept most insurance plans including Medicare, Blue Cross Blue Shield, Aetna, Cigna, and Tricare to name a few. We are accepting new patients at both office locations.
